Dexcom (NASDAQ: DXCM) raises full-year outlook and authorizes up to $1 billion buyback

What happened

DexCom, Inc. (NASDAQ: DXCM) raised its 2026 outlook after third-quarter revenue grew 11% to $1.34 billion from $1.21 billion a year earlier.

Dexcom now expects 2026 revenue of $5.20 billion to $5.25 billion, or approximately 12% to 13% growth. It raised the revenue midpoint. It also lifted its non-GAAP operating margin outlook to approximately 24% and its adjusted EBITDA margin outlook to approximately 32%. It reiterated a non-GAAP gross margin of approximately 64%.

The board also authorized repurchases of up to $1.0 billion of common stock, ending no later than Dec. 31, 2027. The filing says Dexcom does not have to buy any set amount. It can extend, change, pause or end the program at any time.

Jake Leach, Dexcom's president and CEO, said the quarter delivered "strong revenue growth and margin progression." He said the results let the company again raise its full-year revenue and operating margin guidance.

Where the growth came from

The U.S. did most of the work. U.S. revenue grew 14% to $969.8 million, adding $117.9 million. International revenue grew 5% to $374.7 million, adding $17.3 million. Organic growth matched the reported rate overall, and international organic growth was also 5%.

The pattern held over nine months. Revenue through Sept. 30 rose 13.0% to $3.84 billion from $3.40 billion a year earlier, a $442.4 million increase.

Why it matters

OptimistFi has argued that Dexcom compounds if continuous glucose monitoring keeps becoming standard infrastructure and the company defends its sensor economics. Gross margin is the closest read on the second condition, and it rose.

GAAP gross margin was 64.3% against 60.5% a year earlier. Cost of sales was $480.1 million versus $477.9 million while revenue climbed. Gross profit rose $133.0 million on a $135.2 million revenue gain. Nearly all of the added revenue reached gross profit.

That carried into operating income, which reached $327.2 million against $242.5 million. The GAAP operating margin rose to 24.3% from 20.1%, a gain of 420 basis points. Operating expenses rose to $537.2 million from $488.9 million, slower than revenue.

On the first condition, Dexcom says it published the CONNECT randomized clinical trial in The New England Journal of Medicine after the quarter ended. It says the results redefine the standard of care for people with Type 2 diabetes not on insulin. It also launched a redesigned Stelo experience and won its first two international clearances for the Dexcom G7 15 Day System.

Where the profit lines diverge

Below the operating line, the picture is weaker. GAAP net income was $275.1 million, down from $283.8 million. Diluted earnings per share still rose to $0.72 from $0.70, because the diluted share count fell to 383.5 million from 407.4 million.

Other income, net fell to $7.5 million from $109.8 million. Income before income taxes slipped to $334.7 million from $352.3 million even as operating income rose.

Part of the GAAP margin gain also reflects smaller one-time charges. Third-quarter 2025 operating income carried $22.7 million of business transition and other items, mainly workforce reduction costs and rent on vacated San Diego office space. This year the figure was $0.4 million. The non-GAAP operating margin, which excludes those items, rose a smaller 270 basis points to 25.3%.

On a non-GAAP basis, net income rose to $275.0 million, or $0.73 per diluted share versus $0.61 a year earlier.

What the buyback has to work with

Dexcom held $1.82 billion in cash, cash equivalents and marketable securities on Sept. 30, and its revolving credit facility was undrawn. Long-term senior convertible notes stood at $1.24 billion.

Treasury stock at cost rose to $3.13 billion from $2.08 billion at Dec. 31, 2025. The new authorization adds to that pool only if Dexcom chooses to use it.

What's next

Management holds its conference call at 4:30 p.m. Eastern on Oct. 29. The next hard test is Dexcom's next quarterly report.

Subtracting nine-month revenue from the guidance range leaves about $1.36 billion to $1.41 billion for the fourth quarter, by OptimistFi's calculation. A result below the low end would weaken the case that the raised outlook holds. Gross margin staying near the approximately 64% non-GAAP guidance would support the sensor-economics side of OptimistFi's view.
