What happened
Centene Corporation (NYSE: CNC) raised its 2026 adjusted earnings guidance by $0.40 a share. It also said debt has fallen $1.1 billion since June 30.
For the quarter ended Sept. 30, Centene posted adjusted diluted earnings of $0.58 a share and a GAAP loss of $0.14. For the full year it now guides to adjusted diluted EPS of greater than $5.20 and GAAP diluted EPS of greater than $3.46.
Management also lowered its full-year health benefits ratio (HBR) range to 90.4% to 91.1% and its interest expense range to $600 million to $620 million. The HBR midpoint improved by 15 basis points to 90.75%. HBR is medical costs divided by premium revenues, so a lower ratio leaves more premium for other costs and profit.

Third-quarter results
Premium and service revenues rose 0.5% to $45.1 billion from $44.9 billion a year earlier, a gain of $207 million by OptimistFi's calculation. The filing credits premium yield and membership growth in Medicare prescription drug plans, rate increases in Marketplace and Medicaid, Marketplace risk adjustment revenue transfer and state directed payments. Lower Marketplace and Medicaid membership offset part of the gain.
The GAAP loss compares with a loss of $13.50 a year earlier, when Centene booked a $6.72 billion goodwill impairment. The adjusted figure compares with $0.50 in the third quarter of 2025. The GAAP SG&A expense ratio rose to 7.2% from 7.0%. The filing ties that to severance and vendor costs, including $276 million of severance. The adjusted ratio fell to 6.5% from 7.0%.
Employees numbered 53,900 at quarter end, against 60,900 a year earlier.

Where margins stand by business
The third-quarter HBR was 92.7%, unchanged from a year earlier. Centene said that was better than expected, driven by the Medicare segment, with Medicaid and Marketplace on track.
The filing says last year's ratio was helped by a decrease to the premium deficiency reserve for Medicare Advantage. There was no such benefit in 2026. This year's ratio benefited from a lower Marketplace HBR on improved pricing and risk transfer, and from Medicare Advantage and prescription drug plan performance.
Medicaid HBR was 93.6% against 93.4% a year earlier. Commercial improved to 89.0% from 89.9% and Medicare to 93.8% from 94.3%. Medicare premium and service revenues rose 20% to $11.26 billion and Medicaid rose 4% to $24.04 billion. Commercial fell 21% to $8.70 billion.
Over nine months the HBR was 89.9% against 91.1% in 2025. The full-year guidance range sits above that nine-month figure. Adjusted EPS for the nine months was $6.45, already above the new full-year floor.
Why it matters
OptimistFi has argued that Centene works only if Medicaid and Marketplace growth can be repriced into underwriting profit after the 2025 margin break. The quarter supports that view in part. Marketplace margin improved on pricing, and Centene says Medicaid is on track. Nine-month GAAP net earnings were $2.56 billion against a loss of $5.57 billion a year earlier, a period that included $6.80 billion of impairments.
The evidence is thinner on Medicaid, the largest business. Its HBR edged up and total membership fell to 11.9 million from 12.7 million. Marketplace membership dropped to 3.4 million from 5.8 million, so the pricing gains apply to a smaller base.
Cash flow used in operations was $3.7 billion in the quarter. The filing attributes this to the timing of pass-through, premium and other payments, as expected. Nine-month operating cash flow provided was $4.29 billion against $4.65 billion in 2025.
Debt and cash
Centene repurchased $558 million of senior notes due 2027 and 2028 during the quarter. Total debt stood at $15.5 billion, with no borrowings on its $4.0 billion revolving credit facility. Debt to capitalization was 41.1% at quarter end against 45.5% a year earlier.
In October the repurchase program was increased by $2.0 billion and Centene bought the remaining $569 million of its 2027 notes. That left $1.9 billion available under the program as of Oct. 27.
Nine-month interest expense was $466 million against $510 million a year earlier. Cash, investments and restricted deposits totaled $39.9 billion. The company kept $175 million of cash available for general corporate use.
What's next
Chief Executive Sarah M. London said the company is focused on a strong finish to 2026 and on building on this progress in 2027. Centene's next quarterly report is the test of that claim.
A full-year HBR inside the guided range and adjusted EPS above the raised floor would confirm the repricing the case depends on. A ratio above the range, or a Medicaid HBR that keeps climbing, would weaken it.
